Objective Zero User Survey

The Objective Zero Team is currently working with the Centers for Disease Control and Prevention (CDC) and CDC Foundation to understand the effectiveness of the Objective Zero App. 

You’ve been randomly selected to provide feedback about your Objective Zero experience. Objective Zero App users who complete a 10-15 minute phone interview will receive a $20 Amazon gift card for their time. Your feedback will help us make this platform better for other veterans. 

We have several trained interviewers so you can speak with someone who you feel comfortable talking with. These interviewers have also completed our Ambassador training, but have not connected with any users on the app, to ensure anonymity and confidentiality.

Any feedback you share will be confidential, and we will never share your name or contact information. Your comments will be consolidated with feedback from other Objective Zero app users in any summaries shared with the Objective Zero leadership team and the CDC Foundation. 

The Objective Zero Foundation is required to document any survey incentives provided during this project. We will ask you for your digital signature to acknowledge that you’ve received your gift card. Your name will not be attached to your survey answers and this form will be used for audit purposes only.
